You either closed the offer out and returned later trying to mark the offer before clicking visit the offer, clicked the back arrow after previously spinning the wheel, or clicked too many times when you marked it as complete causing it to mess up (making it look like a duplicate or you didn't complete an offer). I'd say it's the last reason, you clicked mark as completed too many times while it was trying to load. It's hard to tell what the other warning message in red says underneath the Spin2win pop-up, it's not the same message. Either way you're not going to be getting that spin so just close it out. Make sure you're just clicking the mark as completed button once even if it appears to be loading slow.
