If you have a reward pending and it doesn't appear in the rewards gallery, does that mean I should cancel and re-order?

You could cancel and re-order. Or you can just wait and see what happens.

If you wait, one of two things will happen: (1) Your order will be approved like normal (2) Your order will be denied and you will instantly be refunded all the points you spent. I would just wait if I were you. Hope your order approves, and SOON. :)
